All PDFs can be filled online and emailed to contact@stjosephmissionschool.org and will be printed for our student files.

Whether filling out online or hard copies, the following items are also required to accompany your Registration Packet:

Birth Certificate
Social Security Card
Immunization Records
CIB or Tribal I.D.
Baptismal/Holy Communion/Confirmation Certificate
Last report card with evidence of promotion from the last school attended.
